Office Policies



No appointments are needed for blood work or vaccines.

No one will be seen without an updated, current insurance card

Physical exams are due yearly.

Please check your discharge paperwork for follow up instructions specific to you!


Missed Appointments

Should you find it necessary to cancel or reschedule an appointment, we ask that you give the office at least one business day notice in advance so we can fill the spot with another patient who is waiting to see the doctor.  By doing so it will ensure that no $50 missed appointment fee will be sent out by our billing company. We do make every attempt to confirm appointments 24 hours in advance.

Late Policy

In fairness to all our patients, if you arrive more than 10 minutes late for your appointment we may need to reschedule your visit.


Co-payments and deductibles will be collected at the time of service and are based on your contract with your insurance company. Failure to pay at the time of visit will incur a surcharge of $25 from our billing company. If a personal check is returned for any reason, you will be charged a $25 service fee.

Prescription Refills

We ask that you get  your prescriptions renewed during your routine office visits, however we will refill prescriptions by phone when needed. Refills are done Monday - Friday during office hours. If approved, please give the doctor 48 business hours to process your request for refills.  If your prescription is being called or faxed to your pharmacy, please check with the pharmacy 48 hours after your request before calling the office back. Patients are required to see Dr. Lerner on a regular basis in order to qualify for medication refills. Patients who are unable to keep scheduled appointments may be granted a limited supply of medication.

Narcotic Prescriptions/Controlled Substances

ALL patients on chronic narcotic prescriptions or on controlled substances will need to have an appointment for renewals. Patients will be required to sign a "patient contract" and may be subject to random urine testing. Patients who have substances in their urine not prescribed from our office or do not have substances that we are prescribing may jeopardize their refill privileges.


As your primary care office, we will be responsible for completing all of your insurance referrals to specialists, hospitals, therapy and any others required by your insurance provider. We ask that you give us 72 business hours to complete your request.  Please make sure to give us: the provider that you need the referral for , providers ID/NPI number, reason for seeing the provider, providers fax number and the date of your appointment. If your insurance company requries a referral, please ensure that your referral request is submitted to our office 72 business hours in advance so we can process it.

Prior Authorizations

Insurance companies frequently deny payment for certain non-formulary prescriptions. To avoid the confusion of dealing with these requests, it is very important that you supply Dr. Lerner with a copy of your formulary (preferred medications your insurance plan will cover). In the event that a prior authorization is required, it is your responsibility to contact your insurance company to obtain an updated formulary and all appropriate paperwork. Dr. Lerner will make every effort to conduct a "prior authorization" on your behalf so that you can receive your medication. This process may take a few weeks. An office visit may be required to address "prior authorization" concerns and to help you fully understand all options available to you.
